How long is my card good for?
Your card is valid for one year. You can renew your card at any Family PACT provider location.
How do I renew my card?
Your Family PACT eligibility has to be recertified each year. The recertification and eligibility process is completed through a Family PACT provider. Any Family PACT provider can help you complete the client eligibility process and recertify your Health Access Card if you continue to be eligible for the program.
To find a provider near you, enter your zip code in the zip code locator at the top of the website or call 800-942-1054. This will give you providers to call in your area for an appointment.
Notify your provider of any changes to your to your information on record. Your provider will let you know if a change has impacted your eligibility for the program. If your income increases above the program level due to a new situation, you would no longer be eligible for Family PACT.
As a Family PACT client, you are never to be billed for Family PACT benefits. However, always verify your eligibility before a procedure to assure you still qualify for Family PACT and your HAP card is current.
