Preconception Care:  New Guidelines, New Opportunities

• To simply receive a certificate of participation, fill out, print and fax the Evaluation form when you complete the session.

• If you want CE credit, fill out the CE application form, the Evaluation form and the Post-test and fax them at the same time to receive your CE certificate.

• Before you start the session, print out the Evaluation form (all participants) and Continuing Education form and Post-test (if needed).
