Family PACT providers have access to a wide range of easy-to-read, culturally appealing client education materials. These materials are available to Family PACT providers AT NO COST to them.

As a Family PACT provider, you can order these materials by following these three easy steps:

1. Decide what materials you need in your setting.
Read the Family PACT Catalog of Client Education Materials (PDF, 2MB).

2. Print and fill out the Family PACT Order Form (Updated 5-08) (PDF).

3. Fax the Family PACT order form to 1-888-723-3667.

NEW Downloadable Materials

Family PACT is making newly updated and newly developed materials available online.  As a Family PACT provider, you can print these materials yourself as you need them.  Each new title is now available in 6 different languages -- English, Spanish, Chinese, Vietnamese, Korean and Russian.
