Family PACT providers have access to a wide range of easy-to-read, culturally appealing client education materials. These materials are available to Family PACT providers AT NO COST to them.
Published Client Education Materials
As a Family PACT provider, you can order printed materials by following these three easy steps:

Decide what materials you need in your setting.
Read the 
Family PACT Catalog of Client Education Materials (PDF, 6MB).

Print and fill out the Family PACT Order Form (Updated 08-10) (PDF).

Fax the Family PACT order form to 1-888-723-3667.
Your order will be shipped to you. If you have any questions about your order, call 1-800-541-5555.
Sterilization Materials
Sterlization Consent Form Ordering
Sterilization Consent Forms (PM 330) (English on one side, Spanish on the other) may be ordered by calling the Telephone Service Center (TSC) at 1-800-541-5555. Providers must supply their NPI number when ordering the form(s). The following information also may be requested:
- Date of request
- Name of document (Sterilization Consent Form, PM 330)
- Registered provider name associated with the NPI
- Ship to site address (service address) (P.O. Box not accepted)
- Quantity of forms requested
- Contact person and telephone number
Sterilization Booklets
Informational brochures on male and female sterilization entitled
Permanent Birth Control for Men and Women are available for download in English and Spanish on the DHCS website.
Ready to Use Client Education Materials
Don't want to wait for your warehouse order? Family PACT has made various topics available below. As a Family PACT provider, you can print these materials yourself as you need them. Each title is available in 6 different languages — English, Spanish, Chinese, Vietnamese, Korean and Russian. Many of these titles may also be ordered from the catalog.
Birth Control Methods
These materials are intended to help you provide appropriate informed consent for your clients regarding their chosen birth control method.
Sexually Transmitted Infections
There are pamphlets intended for clients who have been diagnosed with a sexually transmitted infection as well as information flyers you can use in education and counseling settings or in outreach efforts.
Outreach Flyers and Resources
You can use these materials in the schools, or in your community. Current topics include Birth Control, STIs, and Mammograms. Also available is a panel card that explains Family PACT services.
