Enrolling as a Family PACT Provider

To Be a Family PACT Provider:

1. You must be a current Medi-Cal provider in good standing.
2. You must attend one of the upcoming Provider Orientation Sessions.
3. You must submit completed Application and Enrollment Agreement forms. To download these forms, go to Provider Enrollment Forms.
