Course Description

This session provides information on which clients are eligible and which clients are ineligible for Family PACT program services. How to assess a client’s eligibility for the Family PACT program and how to accurately complete the Client Enrollment Certification form is also discussed. The three methods of activating, reactivating and deactivating a client’s Health Access Program (HAP) card are also reviewed.

Course Objectives

At the conclusion, participants will be able to:

  1. Describe which clients are eligible and which clients are not eligible for Family PACT program services.
  2. Describe how to assess a client’s eligibility for Family PACT program services.
  3. Accurately complete a Client Enrollment Certification form.
  4. Activate, reactivate or deactivate a client’s HAP card through one of the three systems available.

All participants should download an evaluation form prior to viewing the session. Click the link and print a copy of the evaluation. The slide presentation and slide handout is also available. After viewing, fax completed evaluation form to (213) 368-4410, Attn: Family PACT. Once your evaluation is received, you will receive a Certificate of Participation in 2-4 weeks.

To begin session:

Click this link Connect to Webex. On the left column, find the 'Attend a Session' section and then click 'Recorded Sessions.' You'll see the session listed there, click on the title. Complete the registration form. Once registered, click the playback arrow to start the session. After the recording starts playing, you can open the interactive panels. First, click on the 'View' menu. Then select 'Float All Panels.' This will allow you to see all the panels as they were used and viewed during the live webcast of the event.
