Course Description
Using a case study approach, this webcast will report findings of the 2007 MRR, provide examples of good charting, and offer recommendations and resources to improve both the quality of clinical care and of medical record documentation.
Course Objectives
As a result of this training, participants will be able to:
- Describe one omission noted in medical record documentation for each of four types of client encounters.
- List at least one finding associated with each of four types of encounters regarding the quality of documentation found in the 2007 Medical Record Review (MRR).
- List three advantages of using "check-off" medical record forms.
- List three interventions that are available when the clinician and the client are language discordant.

This is an activity offered by the California Family Health Council, Inc., a CMA-accredited provider. Physicians attending this program can earn up to 1.5 contact hours of Category I credits toward the California Medical Association’s Certification in Continuing Medical Education and the American Medical Association’s Physician's Recognition Award. The American Academy of Physician Assistants and the American College of Nurse Midwives accept Category I CME approval.

CFHC is designated as a provider of Category I continuing education contact hours in health education by the National Commission for Health Education Credentialing, Inc. Health Educators attending this program may report up to 1.5 Certified Health Education Specialist (CHES) contact hours.
The California Family Health Council, Inc., provider number PCE3165, is an approved provider of continuing education for MFT and LCSW licensure requirements in California. Course meets the qualifications for 1.5 hours of continuing education credit for MFCCs and/or LCSWs as required by the California Board of Behavioral Sciences.
