Improving the Quality of Chart Documentation: Findings from the 2007 Medical Record Review

Course Description

Using a case study approach, this webcast will report findings of the 2007 MRR, provide examples of good charting, and offer recommendations and resources to improve both the quality of clinical care and of medical record documentation.

Course Objectives

As a result of this training, participants will be able to:

  1. Describe one omission noted in medical record documentation for each of four types of client encounters.
  2. List at least one finding associated with each of four types of encounters regarding the quality of documentation found in the 2007 Medical Record Review (MRR).
  3. List three advantages of using "check-off" medical record forms.
  4. List three interventions that are available when the clinician and the client are language discordant.
